The history of electric cardiac stimulation is long and fascinating. The grow- ing clinical importance has been recognized and renewed as Zoll in 1952 described a successful resusciation in cardiac standstill by external stimula- tion. Meanwhile, patients with disorders of cardiac rhythm have been treated all over the world for about 15 years. Recently the technical refinements of intracardiac stimulation in combination with a subtile analysis of endocavi- tary electrocardiograms improv~d the understanding of human intracardiac conduction. These new procedures include the possibility to study the initia- tion and termination of tachycardias. - The pertinent interest in these devel- opments and the intention to learn more about the basic fundamentals of excitability and conduction under both normal and pathological circum- stances were reasons to organize an international symposium on that topic. - The papers contained in this book were presented at this meeting on diagnos- tic and therapeutical tools of cardiac pacing, held at Munich on November 7 and 8, 1975, sponsored by the first Medical Clinic of the University of Munich and generously supported by Medtronic. Especially focused are the investigations on sinus node function, disturbances of av-conduction and electrophysiology of supraventricular and ventricular tachycardias. The content of the book that is emerging from the symposium were created by the authors. I wish to thank them for teaching us about the recent advances in cardiac tissue stimulation by electricity. Much credit is due Priv. Doz. Dr. B. Uideritz, who organized the symposium and published these proceedings.

Throughout the past decades, the understanding of the pathophysiology of chronic heart failure and the therapeutic approach to this condition have un dergone considerable change. New developments are reported every year. Perhaps it is because of this continuing interest of researchers and physi th cians that the chapter on chronic heart failure of the 4 edition of Klinische Kardiologie has been so positively received. Several colleagues asked the authors to publish the chapter on chronic heart failure separately, because it would be more practical to use in daily work. When the request for an En glish version of this chapter reached the authors and Springer-Verlag, we were happy, on the one hand, because of the interest, but did wonder whether it would be wise to translate a typically German textbook into Eng lish. We invite you to send us your critical comments. Although Guidelines for the Treatment of Heart Failure have been pub lished recently by the WHO, by the European Society of Cardiology, and by the American Heart Association Task Force clinicians, practitioners, and stu dents still seem to need , a comprehensive view with emphasis on pathophys iology, epidemiology and therapy. The authors are grateful to Springer-Verlag for the good co-operation and tremendous personal commitment in making the English edition of Chronic Heart Failure possible. We hope to give our readers a good overview on this important clinical syndrome, one which re quires intelligent clinical research and sound clinical practice as weIl.

More than 10 years have passed since the memorable symposium on "Myocardial Contractility," edited by R. D. Tanz, F. Kavaler and J. Roberts (New York and London, Academic Press, 1967). PathogeneSis of myocardial failure still involves many questions. The latest scien tific findings on fundamentals of myocardial contraction encouraged us to organize this international symposium held in Rottach-Egern at Tegernsee (Germany), June 17 to 19, 1976 sponsored by the European Society of Cardio ogy. It seemed appropriate to assemble prominent workers in this field in an attempt to correlate their respective in formation on cardiac function. In this connection it must be remembered that our present understand ing of the cardiovascular system and today's therapeutic and preventive measures are the fruits of yesterday's research. Further progress in this field will be conditioned by various circumstances: to win highly motivated creative people for clinical research, to mediate time and contacts for their learning new methods, and to provide adequate faci lities for scientific work in our hospitals. Therefore, the aim of the conference was to discuss those aspects of myocardial failure, that are believed to require further studies in the future by integrated efforts of research workers in several disciplines, especially to promote the pertinent exchange of ideas between basic and clinical research."

"Schock und Kollaps" war der Titel des Beitrages von E. BUCHBORN im Band IX/1 der 4. Auflage dieses Handbuches der inneren Medizin, der 1960 erschienen ist. Damals wie heute geht man davon aus, daB in den meisten Hillen ein zirkulatorisches Versagen als die U rsache der im Schock zu beo bachtenden meta- bolischen Organfunktionsstorungen anzusehen ist (Beispiele: der kardiogene und der hypovoUimrsche Schock). Abweichend von dieser gangigen Vorstellung setzt sich neuerdings immer mehr das Konzept durch, daB es im Verlaufe von metabolischen Krisen und von endogenen wie exogenen Intoxikationen auch durch direkte Schadigung des Zellstoffwechsels zu dem Syndrom eines Multi- Organversagens kommen kann. Bezuglich der Schockursachen, der Vorgange der Kreislaufregulation und der Auswirkungen einer Plasmasubstitution im Schock sei auf die schon er- wahnte Darstellung in der 4. Auflage verwiesen; dies gilt auch fUr die Entstehung und Auswirkung der Lactacidose. 1m vorliegenden Band wurde deshalb auf eine inhaltliche Wiederholung dieser heute noch giiltigen Deskription wie auch auf eine lehrbuchmaBig luckenlose Darstellung dieses Stoffgebietes bewuBt ver- zichtet; die hier behandelten Themen befassen sich mit der aktuellen Thematik des kardiogenen Schocks, mit den Auswirkungen spezieller Schockformen auf die Mikrozirkulation und Blutgerinnung, auf Entstehung, Erkennung, Verlauf und Behandlung des, akuten Nieren- und Lungenversagens, mit den endokrinen Faktoren der Schockpathogenese und mit den Problemen einer rtosologisch aus- gerichteten Differentialtherapie. Herausgeber und Autoren danken dem Springer-Verlag fUr die bewahrte Zusammenarbeit, fur die sachkundige Beratung und Geduld bei der Herstellung dieses Bandes.

Seit dem Erscheinen der 4. Auflage des Handbuchs der inneren Medizin (Band IX/1) im Jahre 1960 sind 23 Jahre vergangen. Damals wie heute basiert das pathophysiologische Konzept der chronischen Herzinsuffizienz auf den phy- siologischen Grundlagen der Herz- Kreislauf- Dynamik, determiniert durch Vor- last, Nachlast, Kontraktilitat und Frequenz des Herzens in Verbindung mit den hamodynamischen, metabolischen und endokrinologischen Folgestorungen der Organperipherie (SCHWIEGK u. RIECKER 1960). Auch die von LINZBACH (1960) als "GefUgedilatation" beschriebenen Strukturveranderungen im Myo- kard vermitteln heute noch ein giiltiges morphologisches Paradigma. Neue Untersuchungsmethoden (z.B. Echokardiographie, Ventrikulographie, Coronarographie, Myokardszintigraphie, Myokardbiopsie) haben unsere Kenntnisse aber iiber die Nosologie der chronischen Herzinsuffizienz wesentlich vertieft und spezielle Verlaufsformen der Herzinsuffizienz (z.B. Herzvitien, pri- mare Kardiomyopathien, koronare Herzkrankheit, rhythmogene Herzinsuffi- zienz, chronische Perikarditits etc.) einer kausalen Therapie (z.B. durch herzchi- rurgische Ma13nahmen) zuganglich gemacht. Neben der konventionellen Glyko- sidtherapie haben neue inotrope Substanzen, Diuretika, Antiarrhythmika und Vasodilatantien und praventive Ma13nahmen (z.B. antihypertensive Therapie) das Spektrum der symptomatischen Behandlungsma13nahmen wirkungsvoll er- weitert. 1m Interesse der Aktualitat und Lesbarkeit wird in dem nun vorliegenden Handbuchband bewu13t auf eine inhaltliche Wiederholung der vorausgegange- nen Auflage verzichtet, ebenso auf eine umfassend liickenlose Themenfolge wie auch auf eine lehrbuchma13ige Wiedergabe dieses Gegenstandes; diese Monogra- phie zielt vielmehr auf eine selektiv vertiefende Darstellung vornehmlich der neueren Erkenntnisse der Pathophysiologie, Nosologie, Diagnostik und Thera- pie der chronischen Herzinsuffizienz mit besonderer Beriicksichtigung spezieller Verlaufsformen der Herzinsuffizienz und kausaltherapeutischer Gesichtspunkte.
